  Please note, form development is also available via Microsoft Access; however, due to its potential for macro creation (a computer language code that may be blocked by virus protection software), email transactions are not available.

Upon receipt of your payment and fax or email submission, your order will be processed within 2-3 business days.

*You may purchase merchandise from this Web site by using any one of the payment options listed at www.shop.filla-formz.com. Additionally, turn-around time for project completion is based on the customer's written request, number of pages and the complexity of the form(s); however, forms that are 3 pages or less with low complexity generally will be emailed to the customer within 2-3 business days after receipt of payment. Additionally, all customer e-files are archived for 2 years. Customers may request a free duplicate e-copy of their e-file from the archives of Filla-Formz for up to 2 years (limited to 3 duplicate e-copies per year, per receipt).  All requests must be accompanied by a valid receipt number, verifying proof of purchase.  Note: e-copy represents an electronic copy and e-file represents an electronic file.
